I Want to File a Consumer Complaint
If you would like the Department of Insurance to investigate your complaint against an insurance company, producer or insurance agency, you may complete a consumer complaint form either electronically or in hard copy format. When we receive your completed form, we will initiate an investigation, assign a complaint file number and notify you. Please use this number on all future correspondence with this Department. If you are a health care provider, please use the Health Care Provider Complaint Form.
Please allow four to six weeks for completion of our investigation.
