If you would like the Department of Insurance to investigate your complaint against an insurance company, producer or insurance agency, you may complete a consumer complaint form either electronically or in hard copy format. When we receive your completed form, we will initiate an investigation, assign a complaint file number and notify you. Please use this number on all future correspondence with this Department. If you are a health care provider, please use the Health Care Provider Complaint Form.
Please allow four to six weeks for completion of our investigation.>

Attention: Complaints filed with the Department are confidential records and will not be released to any person or organization except the policyholder, insured or enrollee (or their designee) who originated the complaint or the party against whom the complaint has been filed.
