Consumer General Help
This website is to help consumers locate unpaid life insurance policies associated with individuals that are deceased.
Although the process will be exponentially slower, you do have the option to complete a form and mail it in. If you want to complete the process online you will need to make an account. Creating an account will allow you to track the progress of your request and will store all of the information in a centralized location.

An editable and printable form is located here...IL DOI Policy Locator Form
Here is a prefilled example of the form...Example IL DOI Policy Locator Form
Here is a step-by-step walkthrough guide that will show you everything you need to do in order to complete a request...Walkthrough
There are approximately 500 insurance companies that will be sent each request. The companies have 60 days to respond to each of the inquiries. In most cases, it will not take that long to start seeing responses, but please allow the full 60 days to ensure every company has responded.

If there are any further issues after the 60 days please email us at DOI.LifePolicyLocator@illinois.gov

IMPORTANT: Life insurance companies will respond directly to you ONLY IF the deceased has individual policies or contracts with them AND IF you are authorized to receive this information.
Please feel free to email us at DOI.LifePolicyLocator@illinois.gov.
